Students and staff must wear a mask while inside the school building. Please make sure students arrive at school with a mask each day. If you need masks for your child, please contact the school's principal.

COVID-19 Vaccine Interest Form for Families

If you have a child between the ages of 5-11, the Surry County Health and Nutrition Center would like to know your interest level in a COVID-19 vaccine. This is not a form for an appointment or shot, but rather a way to gauge our community's interest. This will help the health department get the appropriate number of vaccines when they become available for this age range.
